All volunteers will need to fill out a form authorizing The Children's Museum of Indianapolis to perform a limited background check. Please note that The Museum has gone to a new online system. You will receive an email with a link to The Museum's new online system after you fill out this registration form.

If your company performs background checks as part of the employment process, then the Museum will accept a letter on company letterhead from your HR department in lieu of filling out the LCH form.

Volunteers under the age of 18 DO NOT NEED TO FILL OUT THE LCH FORM, however, pleae fill out and submit the Youth Volunteer Form.
